Malaysia scored three goals in the first half, but Thailand leveled the count in the second half, forcing a shootout where all Thai players scored and two Malaysians missed.
Thailand will face Hoang Anh Gia Lai Arsenal JMG, which beat defending champion Vietnam 4-3 on October 25, in the October 28 final.
Earlier, the second-running Australian Sydney team failed to advance to the knock-out round.
